New Students

Entering the Early Learning Program:

1.  Kentucky Immunization Certificate (up-to-date and on the Kentucky form).

2. Birth Certificate (must be a copy of the actual state certificate, not a copy of the registration or announcement form received from the hospital.)
3. Kentucky School Physical (on the Kentucky Department of Educationform and must be dated within 1 year prior to entry).

Entering Kindergarten-8th Grade:

1. Kentucky Immunization Certificate (up-to-date and on the Kentucky form).

4.  Kentucky Eye Exam by Optometrist or Opthalmologist on KDE approved form.
5. Kentucky Dental Exam by a Dentist or trained Health Department RN on a KDE approved form.

**Students that plan to participate in Middle School (6th-8th Grade) Athletics, will need an additional KHSAA sports physical form.  The physical can be completed at the same tiem as the school physical, but must be documented on the proper KHSAA form.

Per KY Statute 214.034(3), All public or private primary or secondary schools and preschool programs shall require a current immunization certificate for any child enrolled as a regular attendee to be on file within 2 weeks of the child’s attendance.

Current Students:
