Holy Name School participates in the federally-assisted National School Lunch Program (NSLP) to provide healthy, balanced, affordable meals to our students. Meals must meet Federal nutrition requirements and the applicable recommendations of the 1995 Dietary Guidelines for Americans; however, specific foods and preparation are determined by diocesan school food authorities.
A very successful project between the U.S. Department of Agriculture (USDA) and the Department of Defense (DoD) has helped provide schools with fresh produce purchased through DoD. USDA has also worked with schools to help promote acquisitions of fresh produce from local, small farmers. In addition to the lunch program, Holy Name's excellent cafeteria staff ensures that a hot breakfast is prepared daily and available from 7am - 7:45am . We also take the extra step of making available mid-morning snacks. Pre-K through the 4 th grades enjoy a grab-and-go snack which includes a pop tart, chocolate milk, and fruit. The 5 th through 8 th grades take a quick power break to include biscuits, gravy, eggs, sausage, French toast, and cinnamon toast depending on the day's menu. A la carte items can be purchased during lunch.

Students entering our 3 year old Pre-School Program will need :
1. Hepatitis B Shot
2. Varicella Vaccine
3. Prevnar vaccine
4. Kentucky Immunization Certificate (up-to-date and on Kentucky form)
5. Copy of the student’s State Birth Certificate (must be a copy of the actual state certificate, not a copy of the form received from the
hospital at the time of birth)
6. Catholic students need a copy of the Baptismal
7. Must be 3 years of age by July lst
Students entering our 4 year old Pre-School Program will need :
1. Hepatitis B Shot
2. Varicella Vaccine
3. Prevnar Vaccine
4. Booster dose of Dtap
5. Kentucky Immunization Certificate (up-to-date and on Kentucky form)
6. Copy of the student’s State Birth Certificate (must be a copy of actual state certificate, not a copy of the form received from the hospital
at the time of birth)
7. Catholic students need a copy of the Baptismal Certificate if not baptized at Holy Name of Jesus Church
8. Second dose of MMR – 4th dose of DPT – 4th Polio Vaccine (last dose after age 4 must be booster dose)
9. Must be 4 years of age by October 1st
10. Physical on a form prescribed by the Kentucky Department of Education and must be dated within 6 months prior to entry
Students entering our Kindergarten will need :
1. Hepatitis B Shot
2. Varicella Vaccine – (2 doses)
3. Kentucky Immunization Certificate (up-to-date and on Kentucky form)
4. Copy of the student’s State Birth Certificate (must be a copy of actual state certificate, not a copy of the form received from the hospital
at the time of birth)
5. Catholic students need a copy of the Baptismal Certificate if not baptized at Holy Name of Jesus Church
6. Second dose of MMR – booster dose of DPT – booster dose Polio Vaccine
7. Physical on a form prescribed by the Kentucky Department of Education and must be dated within 6 months prior to entry
8. Must be 5 years of age by October 1st
Students entering our 6 th grade will need :
1. Recent physical examination on a form prescribed by the Kentucky Department of Education (this physical may be performed and signed
by a physician, and Advanced Registered Nurse Practicioner, or by a health care provider in the Early Periodic Screening Diagnosis
Treatment Program)
2. Varicella vaccine (must have 2 doses if have not already had)
3. Meningococcal vaccine (1 dose)
4. Any child entering 6th grade (OR if their last tetanus was GREATER than 5 years ago) must have the new Tetanus/Pertussis Booster
(Tdap or equivalent) – this vaccine MUST contain pertussis.
Per KY 214.034(3), All public or private primary or secondary schools and preschool programs shall require a current certificate for
any child enrolled as a regular attendee to be on file within 2 weeks of the child’s attendance.
To ensure the health and safety of all children at Holy Name School , only those students who have obtained the above will be allowed entrance into the school at the beginning of the 2011-2012 school year.
